Levitra 2.5-20 mg: what the QT screen actually catches

Last reviewed · 11 min read · Updated

ECG stroke beside the 2.5-20 mg vardenafil range

Vardenafil QT reviews get melodramatic. The label study in 59 healthy men showed small QTcF increases: about 8 ms after 10 mg and about 10 ms after an 80 mg pulse, measured near one hour.

The screen that matters is congenital long QT and Class IA or Class III antiarrhythmics. Those patients should avoid Levitra.

Onset ratings: fast peak board. Panel: vardenafil. Class: PDE5 match.

Board card

10 mg QTcF~8 ms vs placebo at 1 h
80 mg QTcF~10 ms (4× max dose)
Heart rate~+5 bpm at 10 mg
AvoidCongenital LQTS; IA / III AAD
Range2.5-20 mg; usual start 10 mg

The screen is a drug list, then an ECG

Nitrates and guanylate-cyclase stimulators remain contraindicated. A clean QTc does not open nitroglycerin.

Men ask for a 'QT clearance' as if a single clinic tracing blesses every future Levitra swallow. The first catch is the medication list. Quinidine, procainamide, amiodarone, and sotalol are the named Class IA and III examples.

Congenital long QT syndrome is the other named avoid. A normal ECG last year does not delete that diagnosis.

Yuki Tanaka stamps the QT tag before anyone brags about a fast peak on the onset board. Speed is irrelevant if the antiarrhythmic list was never read.

Men who 'feel fine' on sotalol still sit on the avoid list. Symptom absence is not a QT clearance.

A printout from a watch ECG is a conversation starter. It is not the thorough QT study and it is not a full 12-lead.

Eight milliseconds on a 10 mg study hour

The crossover study used 10 mg and 80 mg vardenafil, placebo, and moxifloxacin 400 mg as the positive control. QT was read at one hour because that approximates peak concentration.

Fridericia-corrected QT rose about 8 ms on 10 mg and about 10 ms on 80 mg versus placebo. Heart rate rose about 5 beats per minute on 10 mg and about 6 on 80 mg. Uncorrected QT did not tell a simple 'the drug lengthens QT' story.

Moxifloxacin behaved like a known QT-active control. The vardenafil changes were small. The label still says the clinical meaning of those milliseconds is unknown. That humility is part of the rating.

Healthy men aged 45 to 60 were the study sample. They are not a 78-year-old on amiodarone. Do not export the 'small change' headline to that chart.

Heart-rate rise of about 5 bpm is easy to miss on a clinic pulse and still matters for QT correction method fights. Fridericia and individual corrections were both reported because no single method owns the truth.

Vardenafil 10 mgQTcF ~+8 ms; HR ~+5 bpm
Vardenafil 80 mgQTcF ~+10 ms; HR ~+6 bpm

Clinic ECG after coffee is not the whole tag

A single 12-lead after a double espresso is a noisy baseline. Electrolytes, especially potassium and magnesium, change QTc more than men expect.

Loop diuretics and poor intake can set up a longer QTc before vardenafil is even offered. Fix the copper-bottom labs if they are off.

This board does not treat a one-off tracing as a lifetime vardenafil passport. The drug list still outranks the PDF in the portal.

QTc that lengthens after a diarrheal illness is often electrolytes. Repeat the labs before you retire Levitra forever or bless it forever.

Pre-op ECGs done for other reasons are useful only if someone actually reads the QTc and the med list together.

  • Name IA and III agents every visit.
  • Recheck K+ and Mg2+ if QTc looks new.
  • Do not start vardenafil to 'test the ECG.'

What QT reviews overweight

Millisecond panic outweighs the named avoid list. Eight milliseconds in healthy men is not the same story as sotalol plus 20 mg.

Watch-app tracings outweigh a spoken drug list. Gadgets do not name amiodarone.

Last year's 'normal ECG' outweighs this year's booster or azole.

Forum claims that 'all PDE5 are the same on QT' outweigh Levitra's printed IA/III language. Sameness is not what the label wrote.

Onset bragging outweighs the screen. Speed is a later review. See fast-peak ratings.

Quinidine, sotalol, and congenital long QT

Class IA and Class III antiarrhythmics are the hard avoid. Do not bargain a 'tiny sotalol dose' against a 2.5 mg Levitra. The label language is avoid, not titrate together.

Congenital long QT is the other hard avoid. Family history of sudden death and a known LQTS genotype belong on the intake, even if the man feels well.

Other QT-active drugs (some antipsychotics, some macrolides, some antiemetics) are a clinical judgment, not a second printed contraindication list. Name them aloud. Do not hide them behind 'I only take it sometimes.'

Procainamide is less fashionable than amiodarone and still Class IA. Old bottles in a drawer count.

Family history of sudden death at young ages belongs next to the congenital LQTS question even when the patient has never heard the syndrome name.

What to write down before the first 10 mg

Congenital LQTS: yes, no, or never asked. 'Never asked' is a fail.

Class IA and III agents by name, including old drawers.

Other QT-active drugs taken even twice a month.

Latest K+ and Mg2+ if a tracing looks new or the patient is on a loop.

Nitrate access at home or in a sports bag.

Bring that list. The vardenafil panel is where the interaction caps live.

Watches, loops, and 'I feel fine' on sotalol

Wrist-watch AF alerts do not list amiodarone. Use them as a prompt to speak, not as a Levitra passport.

Loop diuretics plus a poor week of intake can lengthen QTc before any PDE5 is offered. Recheck K+ and Mg2+ when cramps or palpitations show.

Sotalol without symptoms is still a printed avoid. The label did not ask how well you feel on the antiarrhythmic.

Family members who died young of 'a bad heart' deserve a congenital LQTS question even when the patient has never heard the letters.

Macrolide weeks and new antipsychotics are judgment calls. Name the exact drug. This board will not bless a class nickname by inbox.

Pre-op tracings help only if someone reads QTc and the med list in the same sitting. A PDF in the portal is not a screen until that happens.

Nitrate access in a golf bag still outranks a tidy QTc. Ischemia plus PDE5 is a different emergency than milliseconds.

When the list is messy, compare members on the match board with the cardiologist in the room, not with a stopwatch from the onset review.

New loop starts in heart-failure clinic should trigger a potassium look before anyone refills Levitra on autopilot.

Post-vomiting weekends change electrolytes. Delay a first 10 mg until the patient can keep food and the labs make sense.

If congenital LQTS was never asked, the screen is incomplete even with a perfect tracing. Ask the question.

Drawer quinidine from a decade-old atrial-fib story still counts. Old bottles are part of the list.

If the cardiologist already banned PDE5, this board will not reopen that ban because a forum called 8 ms small.

Bring the actual bottle names to the visit. 'A heart rhythm pill' is not a screen. Quinidine and a beta-blocker are different sentences.

If the list is clean and the tracing is only a curiosity, say so. Do not let milliseconds block a man who has no named avoid.

Gatifloxacin taught an additive lesson

A later study combined 10 mg vardenafil with 400 mg gatifloxacin, a drug with a comparable QT effect. The combination added more QTc change than either drug alone.

Additive QT is the practical warning: two small effects can share a night. The clinical impact of that additivity is also listed as unknown. Unknown is not permission to stack freely.

If another QT-active drug is required, the review often moves to a different PDE5 member or to no PDE5. The match board is for that conversation, not a home cocktail.

Gatifloxacin is off many formularies now. The additive lesson is not. Any second QT-active drug can share a night.

Unknown clinical impact is a reason for caution, not a reason to shrug. The label bothered to print the additivity.

Chest pain still outranks a QT chip

An ischemic story plus any PDE5 is a nitrate-access problem. Men who keep nitroglycerin at home should not start Levitra because their QTc looks tidy.

Hypotension with alpha-blockers is a separate vardenafil review. QT did not replace blood-pressure counseling.

Sudden vision or hearing change and priapism stay on the emergency list. The QT tag does not swallow those warnings.

Men who keep 'just in case' nitroglycerin in a golf bag should not start vardenafil after a pretty QTc. Access to nitrates is the issue.

Syncope after a first 10 mg is a stop. Do not call it a fluke and swallow 20 mg the next night.

Why the label used an 80 mg pulse

Eighty milligrams is four times the highest recommended ED dose. The point was to cover concentrations seen when a low vardenafil dose meets ritonavir 600 mg twice daily, the strongest studied CYP3A4 interaction.

That is why ritonavir rules collapse vardenafil toward 2.5 mg and a long interval. The QT study and the interaction table are the same story told twice.

Do not take 80 mg because a paper mentioned it. It is a study probe, not a rung on the 2.5-20 mg ladder.

Ritonavir 600 mg twice daily is an old interaction-study dose. Modern boosters still need the labeled vardenafil cap. Do not freelance because the milligram on your antiviral changed.

Ketoconazole and itraconazole are the other named strong inhibitors that collapse the vardenafil rung. Nail courses last long enough to matter.

What this board stamps after the screen

A complete vardenafil QT review names congenital LQTS, lists IA and III drugs, notes other QT-active meds, and only then talks about a tracing. Onset hopes wait on the onset board. Dose math sits on the vardenafil panel.

Mail ECG or label cites to [email protected]. Yuki Tanaka will fix a page. He will not clear your sotalol by email.

A pretty QTc with a hidden Class III drug is a failed screen. The list still wins.

Onset hopes wait until this tag is written in the chart. Speed is a later hobby.

Soren K., Bay Street My QTc is 430 ms. Is Levitra banned?

Board reply

A single 430 ms value is not the label's avoid list. Congenital long QT and Class IA or III drugs are. Bring the full medication list and any LQTS diagnosis. Ben H., Riverdale Watch said my QTc is fine. Start 10 mg tonight?

Board reply

A watch alert is a prompt to talk, not a Levitra passport. It does not name sotalol, quinidine, or a loop-diuretic potassium dip. Speak the list first. Then decide if a 12-lead adds anything. Patrick O., Hamilton I take sotalol. Doctor said small QT changes are 'clinically irrelevant.'

Board reply

The thorough-QT paper called the millisecond shifts small. The Levitra label still tells patients on sotalol to avoid the drug. Label avoid beats a blog summary of 'irrelevant.' Yusuf A., North York Can I pick tadalafil to dodge the QT issue?

Board reply

PDE5 drugs as a class have QT notes. Vardenafil's label is the most explicit on IA/III and congenital LQTS. Switching members is a prescriber call, not a self-dodge. See the match board. Nia T., Mississauga Why did they study 80 mg if the max is 20?

Lila M., Forest Hill Husband is on amiodarone. Can he try 2.5 mg 'just once'?

Board reply

Class III agents such as amiodarone are on the avoid list with vardenafil. 2.5 mg is not a loophole. Ask about a different ED plan with the cardiologist and the prescriber together. Nora J., Parkdale Uncle died at 32 of 'a bad heart.' I feel fine. Still ask?

Board reply

Yes. Young sudden death in the family is a reason to ask about congenital long QT even if you have never heard the letters. Feeling fine is not the label's avoid criterion.
